Proton radiotherapy: consultation records checklist

1. Provide pathology, staging and a summary of previous treatment to establish radiotherapy goals
2. Prepare the latest original CT or MRI images to compare irradiation fields
3. If previously irradiated, retrieve complete plans and dose files for cumulative exposure assessment
4. Provide concurrent medication and organ function records; for children, add anaesthetic assessment records

Questions to clarify
- Do I meet the eligibility criteria?
- What are the separate medical and coordination service costs without Chinese health insurance or free-drug assistance?
- How are initial treatment, ongoing treatment, return travel and follow-up arranged?
- Which risks require immediate contact with the hospital?
