Zanubrutinib: previously treated CLL/SLL and other blood cancers: consultation records checklist

1. Provide blood, bone marrow or lymph node pathology and flow cytometry reports to confirm the disease subtype
2. Bring TP53, del17p and available molecular tests to support risk stratification
3. List previous medicines and reasons for stopping chronologically, distinguishing intolerance from resistance
