Cadonilimab combination: first-line persistent, recurrent or metastatic cervical cancerEven with negative PD-L1,
a first-line bispecific antibody option is available.

When planning first-line treatment for persistent, recurrent or metastatic cervical cancer, China's cadonilimab bispecific antibody combination may be considered even with negative PD-L1. Wu Xiaohua's team at Fudan University Shanghai Cancer Center has pivotal study experience. You can enquire about the regimen and ongoing treatment budget; patient acceptance and supply must be confirmed.

COMPASSION-16: pivotal 2024 publication
Median progression-free survival
12.7 months versus 8.1 months
Median overall survival
Not reached versus 22.8 months

This is the analysis published in 2024; the comparator was the same chemotherapy ± bevacizumab plus placebo, not other immunotherapy drugs. View the evidence

PD-L1-negative patients can discuss first-line bispecific antibody treatment

In May 2025, China approved cadonilimab with platinum-based chemotherapy, with or without bevacizumab. Negative PD-L1 need not be the sole reason to rule out this pathway.

Include a bispecific antibody combination in first-line discussions

Cadonilimab targets both PD-1 and CTLA-4, and a Chinese randomised phase III study supports its first-line combination use. Not being limited to PD-L1-positive disease does not make it suitable for everyone.

Choose combination medicines in light of your pelvic treatment history

Bevacizumab is optional. Weigh bleeding, fistula risk, prior pelvic radiotherapy and organ function to balance treatment opportunity and safety.

Example for 60kg: RMB 7,565 per bispecific antibody administration; calculate the full combination cost

Estimated for fully self-funded foreign patients without Chinese health insurance or free-drug assistance; 60kg is only a dosing example. Chemotherapy, optional bevacizumab and monitoring are additional, and the budget must cover continued treatment.

125mg × 1 vial
RMB 1,513
Example: 10mg/kg at 60kg
600mg requires 5 vials, costing RMB 7,565 per administration
Platinum-based chemotherapy ± bevacizumab
Additional; to be confirmed against the prescription
Infusions, imaging and management of side effects
An itemised hospital quote is needed

Published by Tungwah on 2026-06-26; not an international patient quote. The example uses whole vials every 3 weeks. Weight, treatment duration and combination drugs determine the total.

Finding treatment that fits you
starts with the reports you already have.

Clarifying your disease type and previous treatment helps establish whether this option could form part of a plan for you.

  • Applicable settingFirst-line treatment of persistent, recurrent or metastatic cervical cancer; assess local treatment first if cure remains possible.
  • Previous medicinesOrganise chemoradiotherapy, immunotherapy and recurrence intervals. First-line results cannot be directly applied to switching after immunotherapy resistance.
  • Safety requirementsActive autoimmune disease, infection or organ damage requires specialist assessment.

Seek disease control and survival benefit while planning toxicity monitoring

On a backbone of the same platinum-based chemotherapy, with or without bevacizumab, COMPASSION-16 compared added cadonilimab with placebo. This cannot establish superiority to other immunotherapy medicines.

Hazard ratio for death
0.64 (95% confidence interval 0.48–0.86)
Common severe adverse events
Neutropenia, leukopenia and anaemia
Women enrolled
445 patients

Ongoing treatment and reassessment are needed. Response is not a reason to stop medication yourself; bispecific antibodies can still cause immune toxicity.

  • Bring records of previous surgery and pelvic radiotherapy fields and doses to inform combination treatment decisions
  • Organise recent imaging, recurrence dates and systemic treatment records to confirm the first-line setting
  • Prepare blood counts, liver and kidney tests and any bleeding or fistula history to assess treatment safety

Can I discuss an immunotherapy combination if PD-L1 is negative?

You can discuss the bispecific antibody combination on this page; approval does not require positive PD-L1. First-line persistent, recurrent or metastatic cervical cancer criteria and safety requirements still apply. If cure remains possible, local treatment should be assessed first.

Must bevacizumab be included?

Not necessarily. Bleeding or fistula risks and prior pelvic radiotherapy particularly require careful consideration; the team should decide based on your circumstances.

Can these results apply after progression on immunotherapy?

Not directly. Provide previous chemoradiotherapy, immunotherapy and recurrence intervals so the team can assess subsequent options. A first-line study does not establish the same benefit from switching after immunotherapy resistance.

Which symptoms should not wait?

Breathlessness, persistent diarrhoea or jaundice may indicate immune-related inflammation. Fever or bleeding requires prompt assessment for infection or low blood counts. Bispecific antibodies can still cause immune toxicity, and response is not a reason to stop treatment yourself.
