Resection removes the tumour with surrounding liver tissue. The planned operation may take a small segment or a larger portion, depending on location and anatomy. Ask how much liver would remain and whether its blood supply and bile drainage would be adequate. A smaller surface lesion and a similarly sized central lesion may require different operations. Discuss the possibility of postoperative liver dysfunction, bleeding and infection, as well as recovery needs. An attractive minimally invasive approach does not remove the need to assess the extent of liver tissue lost.

Thermal ablation uses a needle or probe to destroy tumour tissue with heat. It can be considered for selected small HCCs, including situations in which resection would sacrifice too much functioning liver. Its suitability still depends on safe access and the ability to treat the whole target. Nearby bile ducts or other organs can be vulnerable, and blood flowing through a large adjacent vessel may affect heat treatment. Ask what makes complete treatment plausible in your case, what could prevent it, and how the result would be checked afterward.

A useful comparison explains why one option offers a better balance for your anatomy and liver condition. Ask what would make the team change its recommendation, whether either option is currently unsuitable, and what alternatives should then be discussed. Being unsuitable for resection does not automatically make someone suitable for ablation. Nor does a technically successful local procedure remove the need to monitor for residual disease or recurrence. Keep your priorities visible: recovery, future liver reserve and the ability to attend follow-up belong in the discussion alongside cancer control.
