Dysarthria affects the physical production of speech, for example because the muscles used for speaking are weak. Aphasia affects language, including finding words, understanding, reading or writing. A person can have both. This distinction changes the therapy goal: clearer articulation and voice control differ from rebuilding word use or establishing another way to communicate. Difficulty speaking should not be treated as proof that the person cannot participate in decisions. Assessment and discussion should use suitable communication support and give the person time to express preferences, with family help where appropriate.

A therapist may recommend tailored swallowing exercises, posture or texture changes, communication practice or aids; severe swallowing difficulty may require discussion of alternative nutrition. These interventions have different purposes and are not interchangeable. Do not copy another patient's exercises or thicken every drink without assessment. For communication, the useful outcome may be reliably conveying pain, making a phone call or using pictures to express needs rather than an immediate return to previous fluency. Goals should be measurable and revisited. Caregiver teaching matters because much of the daily implementation happens outside the consultation room.
