Haploidentical haematopoietic stem cell transplantationWhen a fully matched donor is hard to find,
a family member may still be a potential donor.

If no fully matched donor is available, parents, children and other relatives may be haploidentical donor candidates. The Beijing Protocol developed in China expands donor pathways; suitability still depends on disease status and donor–recipient assessment.

Transplant study in a specific adult AML population
5-year overall survival after related haploidentical transplantation
78.2% versus 63.6% with a fully matched unrelated donor; P=0.15, superiority not established
5-year non-relapse mortality
13.8% haploidentical versus 15.7% fully matched unrelated; transplant-related risks remain

2016 retrospective matched comparison, 87 patients per group. Limited to newly diagnosed adults with cytogenetically intermediate-risk AML undergoing myeloablative transplantation in first complete remission. Not generalisable to all leukaemias, relapsed/refractory disease or foreign populations; not a randomised superiority trial. View the evidence

Extend the donor search to family members

The ATG-containing Beijing Protocol established by Chinese teams enables assessment of some relatives as haploidentical donors. A broader donor pool does not mean suitability for everyone or better transplant outcomes.

Include family in the donor comparison

Use HLA matching, donor health and antibody tests to compare relatives with other feasible donors. Faster or cheaper care cannot be promised.

Plan life after transplantation too

Use measurable residual disease, immune recovery and graft-versus-host disease assessments to plan relapse monitoring, infection management and long-term follow-up.

Budget for transplant admission and long-term care together

Leukaemia transplantation needs an individual estimate; thalassaemia prices cannot be substituted. Foreign patients are fully self-funded without Chinese health insurance or free-drug assistance. Include donor preparation, admission and post-discharge management.

Haploidentical transplantation for leukaemia
Complete international patient costs require hospital assessment
Budget components
Donor and recipient tests, collection, conditioning, transplant admission and transfusion support
After discharge
Budget separately for anti-infective treatment, immunosuppression, reassessment and complications
Thalassaemia only: Guangxi public education, 2024
Fully matched sibling RMB 100,000–150,000; unrelated or haploidentical RMB 200,000–250,000

The thalassaemia figures are historical educational ranges, not leukaemia prices or confirmed foreign-patient quotes. Severe infection and intensive care may substantially increase costs.

Finding treatment that fits you
starts with the reports you already have.

Clarifying your disease type and previous treatment helps establish whether this option could form part of a plan for you.

  • A confirmed need for leukaemia transplantationDetermined by subtype, molecular risk, depth of remission and performance status; diagnosis alone does not mean immediate transplant.
  • Ability to support careInfection control, organ assessment and long-term caregiver support are needed; donors undergo separate medical evaluation.
  • A separate thalassaemia pathwayA 2026 study across 16 Chinese centres enrolled 823 patients, including 140 haploidentical thalassaemia transplants; its results cannot predict leukaemia outcomes.

Interpret related haploidentical results according to disease status

The 2016 retrospective matched study was limited to newly diagnosed adults with cytogenetically intermediate-risk AML receiving myeloablative conditioning in first complete remission. There were 87 haploidentical and 87 fully matched unrelated recipients; superiority of haploidentical transplantation was not established.

5-year overall survival
78.2% haploidentical versus 63.6% fully matched unrelated, P=0.15
5-year non-relapse mortality
13.8% haploidentical, 15.7% fully matched unrelated

Risks include relapse, severe infection, graft-versus-host disease and treatment-related death. Study results are not an individual prognosis. The matched AML study above used a Peking University People's Hospital transplant cohort; Peking University's official profile confirms that Huang Xiaojun's team established the Beijing Protocol.

  • Organise bone marrow, flow cytometry, chromosome and genetic reports to define leukaemia subtype and risk
  • Provide all treatment records and the latest measurable residual disease results to assess current status
  • Provide HLA information for the patient and potential relatives to compare feasible donors
  • Complete infection, organ function and medication records and explain long-term caregiver availability

Can family donate if I have no fully matched donor?

Parents, children and other relatives may be haploidentical candidates, but not every relative is suitable. Compare HLA, donor health and antibody results alongside other donors.

Can haploidentical transplantation be guaranteed as good as fully matched transplantation?

No. Disease status, donor and prophylaxis affect outcomes. The specific 2016 AML study on this page did not establish superiority to fully matched unrelated transplantation and cannot promise an individual prognosis.

Can treatment end at discharge so I can return home immediately?

Discharge is not the end of treatment, and a one-month stay cannot be promised in advance. Early after transplant, frequent reviews monitor blood counts, infection and graft-versus-host disease. Do not stop immunosuppressants yourself. Local accommodation and international handover must be arranged.
