Diabetic kidney disease may develop without obvious symptoms. Blood testing estimates how well the kidneys filter, while a urine albumin test looks for protein leakage; these provide different information. Bring dated results and earlier values so the clinician can assess change rather than a single number. Ask whether findings need confirmation, what they imply for medication selection and what follow-up is appropriate. Swelling or a change in how you feel does not reliably measure kidney function, and an apparently normal daily routine does not replace the recommended tests.

A history of a heart attack, heart failure and cardiovascular risk without established disease are not interchangeable situations. Bring relevant discharge summaries, heart imaging reports and the list of blood-pressure, cholesterol and other cardiac medicines. Glucose management is only one part of reducing risk; blood pressure, lipids, smoking and physical activity also matter. Ask the clinician to explain the role of each proposed medicine. Do not add aspirin or discontinue an existing heart medicine simply because a diabetes consultation is planned: its benefit and risk depend on the individual indication.

Some glucose-lowering drug classes can offer cardiovascular or kidney benefits in selected patients, so a review may discuss more than their effect on HbA1c. The choice still depends on the exact heart or kidney problem, other medicines, tolerability and individual safety considerations.
