Alzheimer’s Disease: Turn a Treatment Quote into a Budget You Can Check

After receiving a quote, first ask the treating doctor to specify the assessments or treatment planned for this visit, then ask the hospital to list costs by stage. Calculate diagnostic review, medication and administration, follow-up monitoring, and family care separately. Mark items without a price or number of visits or doses as “To be confirmed”; do not count them as zero in the total.

Address safety first. If the patient suddenly becomes confused or develops possible stroke symptoms, such as facial drooping, weakness on one side, or difficulty speaking, seek emergency help immediately, even if the symptoms later improve. In mainland China, call 120, give your location and describe the symptoms, and follow the dispatcher’s instructions. Stay with the patient while waiting and bring a medication list if possible. Do not delay emergency care while waiting for a quote or appointment.[1][2][3]

First, decide what this quote should cover

Agree with the patient and treating team on the purpose: diagnostic review only, a discussion of symptom management, or consideration of a particular medication after the doctor’s assessment. Ask the doctor to list current recommendations, options that depend on test results, and expected follow-up and monitoring arrangements separately. You can ask the hospital for separate estimates for the basic assessment and care plan and for any additional treatment the doctor considers potentially suitable. The latter is not a promise of treatment eligibility or medication supply.

Prepare previous diagnoses, records of changes in thinking and daily functioning, test reports and readable imaging files, current medications, and records of other medical conditions. First ask whether the treating department can actually review the images and use previous test results, and which tests need to be repeated or added. Assessment may involve medical history, cognitive and functional testing, imaging, and laboratory tests. The doctor should decide whether PET, cerebrospinal fluid testing, or other tests are needed based on the clinical questions; there is no need to add every test to the budget in advance.[4]

Enter the quote in a budget by stage

Ask the hospital’s finance department and pharmacy for an itemized written quote stating the hospital, campus, outpatient clinic type, patient status, currency, quote date, and validity period. For each line, enter “unit price × estimated quantity = subtotal,” and specify the unit, stage, whether it is included in a package, and who charges for it. Leave unconfirmed fields blank. If an item is unnecessary, ask the doctor or billing provider to state why.

Stage Items to request individual prices for Unit price / quantity / subtotal Included items, unknowns, and who confirms
Initial consultation and diagnostic review Consultation, cognitive and functional assessments, review of previous records and imaging, necessary tests To be filled in Department confirms whether needed; finance provides itemized prices
Pretreatment assessment Tests and assessments that may be needed under the doctor’s plan To be filled in Department to confirm specific items, whether existing results can be used, and how long they remain valid
Medication and administration Drug name, dosage form, strength, estimated quantity; infusion or other administration services, supplies, nursing, and observation To be filled in Pharmacy confirms supply, pricing, and unused medication; hospital confirms the scope of administration services
Planned monitoring and follow-up Imaging, follow-up visits, laboratory tests, functional assessments, etc. To be filled in Doctor provides the items and schedule; finance quotes by number of visits or tests
Possible additional medical expenses Unplanned consultations, additional tests, emergency care, or hospitalization To be filled in Triggers and amounts are uncertain; set aside a separate allowance and do not treat these as costs already incurred
Transport, accommodation, and caregiving Round-trip travel for the patient and companion, local transport, accommodation, interpreting, daily care To be filled in Confirm unit prices, number of days, and change and cancellation conditions with each provider
Continuing care after returning to your country or place of residence Consultations, medication, follow-up tests, care, and transport To be filled in Local treating and billing providers must confirm; do not assume care can continue directly

How to calculate: Add confirmed items in the same currency, without double-counting, to get a “quoted subtotal.” List unquoted items and possible additional costs separately alongside it. Separate the first visit, treatment initiation, and the agreed period afterward; do not simply multiply the first month’s cost to estimate a full year. If both the hospital and a service provider quote for interpreting or transport, check service times and what each includes to avoid double-counting. For different currencies, separately record the exchange date, exchange rate, and fees; an unconfirmed exchange rate cannot be treated as the final bill.

Clarify medication, monitoring, and travel arrangements

Medication: Ask the doctor and pharmacy to write down the generic name, dosage form, strength, planned quantity per administration, administration method, and estimated number of administrations at each stage. Ask whether charges are per vial, per box, or based on the amount actually used; how leftover medication is handled and whether you pay for it; and whether the pharmacy can currently supply it. Local regulatory approval, suitability for the individual patient, and the hospital’s ability to supply the medication are separate questions that each need confirmation. Do not choose a dose or frequency yourself based on prescribing information from another country, or estimate an entire course from the price of one vial.

Monitoring: If the doctor proposes anti-amyloid antibody treatment, ask them to specify baseline tests, planned follow-up tests, who will interpret the results, whom to contact about abnormal results, and how additional tests or a treatment pause would affect the schedule and costs. Requirements differ by medication and local protocol; the same monitoring schedule cannot be applied to all treatments. If the doctor discusses a treatment such as donanemab, you can also ask: Does my plan require an assessment such as amyloid PET, and when is it planned? How will the results affect the doctor’s decision to continue or stop medication? Ask the billing provider to list test costs and subsequent medication administration costs at each stage separately so you can update the budget. The responsible doctor decides whether testing is needed and how to act on results under the locally applicable protocol.[9] The US FDA warns that amyloid-related imaging abnormalities associated with lecanemab may involve brain swelling or bleeding, sometimes without symptoms. If headache, confusion, dizziness, vision changes, nausea, difficulty walking, or seizures occur after treatment, contact the treating team immediately or go to the nearest emergency department. Call emergency services immediately for severe or sudden neurological symptoms.[5] Tell emergency personnel which medication the patient is receiving and when the last dose was given.

Travel and care: Ask the treating doctor to assess travel and companion needs. Clarify the expected length of stay, dates for tests and results, intervals between follow-up visits, and who will take over if there are delays. With family members, list costs for transfers, accommodation, interpreting, and daytime and nighttime care. Where needed, separately list hearing and vision support, home safety, and care for other medical conditions. If a particular antibody treatment is unaffordable or not currently suitable, also ask the doctor to discuss symptom management and care arrangements.[6]

Check insurance, payments, and refunds

China’s basic medical insurance drug list, the Commercial Health Insurance Innovative Drug List, and your individual commercial insurance contract need to be checked separately. Ask the basic medical insurance administrator about payment rules for the patient’s status, medication, indication, hospital, and visit date. When checking the Commercial Health Insurance Innovative Drug List, verify the version and policies applicable on that date with the administrator. Then check the specific policy with the commercial insurer. Inclusion in a list does not mean an individual will necessarily receive coverage.[10]

Provide the insurer with written details of the patient, policy, visit date, hospital and department, diagnosis, medication name, and specific services. Ask separately whether coverage includes pre-existing conditions, planned care abroad, medication, tests, administration, monitoring, and hospitalization; whether prior authorization is required; and what conditions apply to deductibles, limits, direct billing, or paying upfront and claiming reimbursement later. After receiving authorization documents, check their validity period, amount, and covered items, and confirm that the hospital has received and accepted them. Continue to mark unconfirmed payment amounts as unknown. Clarify which invoices, itemized bills, prescriptions, medical records, and translations are required for a claim.

Record total costs and upfront payments separately: Total costs are medical and nonmedical expenses without double-counting. Calculate upfront payments at each payment deadline, subtracting only amounts already confirmed and actually available for direct settlement at that time. Do not yet count possible later reimbursement as money received. If items are missing, both calculations will provide only partial subtotals.

Before paying, confirm each recipient’s official payment channels, the purpose of any deposit, final balance settlement, the basis for deductions after cancellation or rescheduling, how to request a refund, and when the money is expected to arrive. Keep written quotes, authorizations, invoices, and proof of payment. Hospitals, accommodation providers, and other third parties each have their own rules. If you also use EastMedGo coordination services, separately check its service information and cancellation and refund terms. The actual written confirmation and applicable rules govern.[7][8]

You can send the hospital this message:

Please list the unit price, unit, estimated number of services, inclusions and exclusions, quote validity period, advance payment, and refund conditions for each service at the planned stages of this visit. Which items must wait until the doctor’s assessment? Can my existing imaging be read and used for this assessment? If additional tests are needed, who will explain the costs beforehand?

Frequently asked questions

Can I calculate a full year’s cost if I only know the medication’s unit price?

No. First, the doctor needs to confirm your individual plan and the number of administrations at each stage. The pharmacy then needs to explain the strength, billing unit, unused medication, and supply. Add administration and monitoring costs separately. Keep all undecided items marked as unknown.

If I have already had an MRI or other tests, will I have to pay for them again?

First submit the reports and images, and ask whether the treating department can read them and whether they adequately answer the current clinical questions. The doctor decides whether further testing is needed. Confirm review fees and the costs of any repeated or additional tests with the hospital.

If the quote includes medication, does it also include infusions and follow-up tests?

Not necessarily. Ask the hospital to state item by item whether medication, administration services, supplies, observation, and follow-up tests are included, to avoid missing or double-counting costs.

If a medication appears on an insurance list, will it be reimbursed?

That alone does not establish your individual benefits. The insurer must confirm coverage in writing based on the policy, hospital, specific services, and dates. A hospital’s acceptance of an insurer also does not mean this treatment has been authorized.

Can I get a refund if a change in the patient’s condition means stopping medication or being unable to travel?

First notify the treating provider and each payment recipient, and stop any new spending you have not yet agreed to. Ask each for a list of costs already incurred, the refundable balance, the basis for deductions, and documentation of the process. Refunds for medical, coordination, and travel services follow their respective agreements.

What happens afterward if I only come to China for one assessment?

Before traveling, confirm with a doctor where you live whether they can receive the medical records, imaging, and treatment plan, and who will be responsible for prescriptions, follow-up tests, and abnormal results. Until a provider has confirmed acceptance, mark follow-up arrangements and costs as pending.

Can I put prices from research papers or old webpages into my budget?

They can help you identify questions to ask, but they cannot serve as a current personal quote. A paper may use a different year, region, treatment duration, or definition of costs. An old webpage also cannot establish that a medication is still available or that an earlier payment policy still applies. Ask the hospital, pharmacy, and insurance administrator for fresh written confirmation.
