Olverembatinib: resistant CML and the T315I mutationContinue CML treatment after resistance,
with a targeted drug for specific resistance mutations.

Adults with repeated CML resistance or a T315I mutation may still have an opportunity to continue targeted treatment with olverembatinib if eligible. Oral treatment requires ongoing reviews based on disease phase, blood counts, molecular results and cardiovascular risk.

Chinese phase I/II study: chronic-phase response data
3-year cumulative complete cytogenetic response rate
69.0%
3-year cumulative major molecular response rate
55.9%

Chinese phase I/II chronic-phase cohort of 127 patients, not all T315I-positive. Single-arm study; cumulative response rates are not an individual's chance of success. View the evidence

Disease phase and mutation requirements must be met. Drug costs reference Ningxia's historical 2023–2024 payment standard, not a 2026 quote. Foreign patients are assessed as fully self-funded without insurance or free drugs. View sources

Turn the next step after resistance into medication and follow-up arrangements

China's domestically developed olverembatinib pathway enables eligible patients to discuss a specific change of medicine and include long-term monitoring in the treatment plan.

Match a change of medicine to the cause of resistance

Review BCR::ABL1 mutations, adherence and disease phase, and compare other TKIs and transplantation. The same results cannot be applied across chronic, accelerated and blast phases.

Plan for treatment continuity

Assess benefit through blood counts and molecular changes, adjusting treatment for cardiovascular risk. Chinese phase I/II research includes several years of molecular and cytogenetic follow-up.

Include long-term medication and monitoring in one budget

Prepare a fully self-funded budget for foreign patients without Chinese health insurance or free drugs. Ningxia's standard for the 2023–2024 agreement period is only a reference for enquiries, not a current prescription price.

Historical payment standard
RMB 239 per 10mg tablet; Ningxia 2023–2024 agreement period
Calculation using the historical standard
At 40mg every other day, 60 tablets over 30 days = RMB 14,340
Current self-funded prescription price
Hospital confirmation needed; the historical standard does not represent a 2026 transaction price
Testing and follow-up
Mutation testing, BCR::ABL1 quantification, blood counts and cardiovascular examinations priced separately

Treatment is usually long term; one bottle is not a complete course. International consultations, accommodation and transport are additional. Any dose reduction is a medical decision.

Finding treatment that fits you
starts with the reports you already have.

Clarifying your disease type and previous treatment helps establish whether this option could form part of a plan for you.

  • T315I-related populationAdults with chronic- or accelerated-phase CML, resistance to previous TKIs and confirmed T315I.
  • Chronic phase without T315IAdults with chronic-phase disease resistant and/or intolerant to both first- and second-generation TKIs can be assessed under the expanded indication.
  • Assess safety firstPrevious cardiovascular or cerebrovascular disease, hypertension and low blood counts require particular attention.

How much response may treatment offer, and what are the risks?

The original 2022 paper documented disease control in resistant patients. Response does not mean cure or that medication can immediately stop. A separate observational study subsequently examined dose optimisation.

Study design
Chinese multicentre, open-label, phase I/II single-arm study
Chronic-phase cohort
127 patients, not all T315I-positive; 3-year cumulative complete cytogenetic response rate 69.0%
Major molecular response
3-year cumulative incidence in chronic-phase disease: 55.9%
Main risks
Thrombocytopenia, hypertension, pericardial effusion and cardiovascular or cerebrovascular events

Responses accumulate over observation time. Percentages from separate studies of other medicines cannot be directly ranked, and these results cannot be extrapolated to blast phase.

Clarify treatment and costs before you travel.

Tell us your diagnosis, previous treatments and main concerns. We help you communicate about Shanghai consultation requirements, budgets and travel arrangements.

  • Bring all bone marrow, blood count and CML phase reports to assess the next treatment pathway
  • Organise BCR::ABL1 quantitative trends and kinase domain mutation reports to identify the resistance issue
  • List each TKI's dose, start and stop dates and reason for stopping to distinguish resistance from intolerance
  • Provide cardiovascular and cerebrovascular history, recent biochemistry and a full medication list to plan safety monitoring

Costs, potential benefit and further treatment deserve specific answers before departure.

Can I continue treatment with this drug without T315I?

Some patients can. The expanded indication covers adults with chronic-phase disease resistant and/or intolerant to both first- and second-generation TKIs; the full treatment history must be assessed.

Can I stop when markers fall and save future drug costs?

Do not stop medication yourself. Long-term treatment and continued molecular monitoring are usually needed. A haematologist separately assesses whether treatment-free remission criteria are met.

Which symptoms cannot wait until the next appointment?

Seek immediate medical care for chest pain, breathing difficulty, sudden limb weakness or significant bleeding, which may indicate cardiovascular events or low blood counts.
