Repotrectinib: NTRK fusion solid tumoursSeek tumour shrinkage after resistance in a fusion-driven tumour,
with the next treatment directed at the cause of resistance.
After previous treatment failure or when no satisfactory alternative exists, adults with NTRK fusion solid tumours who meet the Chinese indication may use repotrectinib to seek tumour shrinkage. In the 2026 single-arm TRIDENT-1 study, the prior-TRK-inhibitor cohort had a confirmed response rate of 48%. This is not an individual's chance of success; the cause of resistance still needs review.

After progression on targeted therapy, keep investigating the resistance mechanism
Repotrectinib has a Chinese adult NTRK fusion solid tumour indication, and TRIDENT-1 separately reported a prior-TRK-treatment cohort. This can inform discussion of subsequent treatment. The US has also approved it; the medicine is not exclusive to China.
Match the medicine to a true fusion
NTRK point mutations, amplification or protein staining alone cannot replace fusion evidence. Tumour-agnostic prescribing still requires individual assessment.
Plan treatment after the previous medicine
After progression on larotrectinib or entrectinib, review the fusion, resistance alterations and lesion sites before comparing next options. Activity against every resistance mechanism cannot be guaranteed.
Plan long-term costs after the initial dose increase
The budget must cover both initiation and maintenance; the first two weeks' cost does not represent the long-term burden. Foreign patients are fully self-funded without Chinese insurance or assistance. The same-drug unit price in 2025 reimbursement submission materials is only a reference.
- Published unit price reference
- 40mg: RMB 49.12 per capsule; not a confirmed international patient price
- 30-day maintenance consumption
- At 160mg twice daily, 240 capsules = RMB 11,788.80
- Initial and subsequent prescriptions
- Once daily for the first 14 days, then escalation; first-month and whole-pack purchase costs require separate confirmation
- Tests and support
- Fusion/resistance testing, imaging, liver function and neurological assessment priced separately
Previous ROS1 insurance information does not establish NTRK reimbursement; no reimbursement is deducted here. Ongoing treatment is needed, with international consultations, travel and toxicity management additional.
Finding treatment that fits you
starts with the reports you already have.
Clarifying your disease type and previous treatment helps establish whether this option could form part of a plan for you.
- Chinese population scopeAdults with NTRK fusion solid tumours that are locally advanced, metastatic or where surgery may cause severe complications.
- Treatment requirementsPrevious treatment failure or no satisfactory alternative; a positive test is not a reason to skip feasible standard treatment.
- Age differencesUS approval includes children aged 12 and over; the Chinese indication verified here is for adults. The US age range cannot simply be applied.
What chance of response remains after previous treatment?
The original 2026 paper reported dedicated NTRK fusion solid tumour cohorts, presenting TRK-naive and previously treated results separately. Responses occurred in previously treated patients, but cannot predict individual outcomes.
- Median duration of response in previously treated patients
- 9.8 months, among responders only
- Important adverse effects
- Dizziness, taste changes, peripheral neuropathy, ataxia and cognitive impairment
- Study limitations
- Single-arm, spanning multiple tumour types; small samples for individual cancers and brain metastases
Intracranial responses were observed, but treatment cannot be guaranteed to replace local therapy. Efficacy and safety require continued reassessment.

- Provide primary tumour pathology, staging and all surgery records to assess feasible treatment pathways
- Bring the complete NTRK fusion report and tissue available for review to confirm a true fusion
- Organise previous TRK medicines, responses and resistance testing to investigate progression after prior treatment
- Provide recent systemic and brain imaging, liver tests and neurological symptom records to plan initiation and maintenance monitoring

Does this regimen also apply to ROS1 lung cancer?
This page covers only China's adult NTRK fusion solid tumour indication and the corresponding cohorts. ROS1 lung cancer approval or response rates cannot be borrowed. The US scope including children aged 12 and over also cannot be transferred to China's adult indication.
Can I take repotrectinib after larotrectinib?
You can discuss it with your doctor, but the cause of progression and indication eligibility must be established. The pretreated cohort's response rate does not guarantee individual benefit. Do not skip feasible standard treatments solely because a fusion test is positive.
Can I take a few boxes, stop and return home, then decide later?
Treatment usually continues until progression or intolerance. Arrange lawful medication continuation and monitoring before returning home. Avoid driving and promptly contact a doctor if dizzy or unsteady. Do not stop or change doses yourself.
Turn the next step after resistance into medication and follow-up arrangements at home.
